A 77-year-old male patient presented for right knee pain. About 2 months earlier he had suffered a comminuted fracture of the right tibial plateau, treated with a plate and screws. He was then prescribed a cycle of rehabilitation to recover the ROM, but after the first three sessions the ROM remained blocked in the range -5°/ +35°. The anamnesis revealed that the patient had undergone right hip arthroplasty 1 year earlier.
HYPOTHESIS:
The edema present at the knee and in the inguinal region hindered the complete recovery of the ROM, for which a dysfunction of the lymphatic system was hypothesized.
MOTOR EXAMINATION:
During the motor examination of the right knee, a soft, elastic and painless endfeel was found, while the hip flexion was very rigid, supporting the initial hypothesis.
PALPATORY VERIFICATION:
A palpatory verification of the knee quadrants revealed edema, while at the hinge quadrant of the right hip there were some swollen lymph nodes and edema of the inguinal region.
TREATMENT:
The inguinal region around the swollen lymph nodes was treated, alternating the hinge quadrant and the distal area of the q-an-me ge and q-an-la-ge. At the end of the treatment, the edema had reduced, and the knee was less swollen.
RESULTS:
Two days after the fascial manipulation treatment, at the next kinesitherapy session the knee range of motion had reached 0°/65°.
